DISEASE MANAGEMENT PROGRAMS

Freedom Pharmacy and Wellness Centers, through its "Circle of Care" programs, provides a non-fragmented approach to the care and treatment of chronic long term diseases. These programs are designed to include all facets of care, treatment and support of the patient, and support of the family and caregivers. The programs promote cost-effective approaches to care. The goal of the entire program is to improve therapeutic outcomes and the quality of life of all of those involved in the disease.

The "Circle of Care" includes the patient, physician, nurse, pharmacist and the caregivers all of whom play a role in assuring that the patient is getting the best care possible at the best price. The pharmacy provides pharmaceutical management, infusions, prescription drugs, research protocols through its sister corporation, the HHCS Research Institute, Inc., prescription products, nutritional supplements and herbal products to help improve quality of life.

Current programs include cystic fibrosis, diabetes, osteoporosis, asthma, COPD, transplant, Cardiac disease, high cholesterol and lipids, high blood pressure, weight management, smoking cessation, cancer, bipolar disorder, Alzheimer's disease and cognitive function, geriatric disorders and men's and women's health.
